Polyvagal Theory (Porges) — Health Applications
Stephen Porges' polyvagal theory identifies three hierarchical states of the autonomic nervous system: ventral vagal (social engagement, safety, connection—rest/digest/connect), sympathetic activation (fight-or-flight mobilization), and dorsal vagal (freeze/collapse/shutdown). Health applications of polyvagal theory recognize that chronic sympathetic or dorsal vagal states produce distinct disease patterns. Chronic sympathetic activation (always in fight-or-flight) drives cardiovascular disease, immune suppression, digestive disorders, and insomnia. Chronic dorsal vagal states (freeze/shutdown) produce depression, chronic fatigue, dissociation, and metabolic slowing. Ventral vagal tone—measured through heart rate variability (HRV)—is associated with better health outcomes across virtually every metric studied.
When to use it
When chronic stress is producing health symptoms and you need a mechanism-based intervention approach. When HRV tracking shows declining autonomic regulation. When standard stress management advice (meditation, exercise) needs a theoretical framework for why it works. When you recognize you're chronically operating in fight-or-flight or shutdown mode.
