Diagnostic Parsimony (Occam's Razor in Medicine)
Diagnostic parsimony is the clinical principle that a single diagnosis should be sought to explain all of a patient's symptoms before invoking multiple diagnoses. A 45-year-old presenting with fatigue, weight loss, and joint pain could have three separate problems — or could have one unifying diagnosis (e.g., lupus). Medical students are taught 'When you hear hoofbeats, think horses not zebras' — common diagnoses are more likely than rare ones. But experienced practitioners know this principle has sharp limits. In elderly, immunocompromised, or multiply-comorbid patients, parsimony fails regularly: the 80-year-old with diabetes, COPD, and depression probably does have three separate diagnoses, not one unifying condition.
When to use it
When diagnosing problems with multiple symptoms or manifestations, deciding whether to seek a single root cause or accept multiple independent causes, evaluating whether a simple explanation adequately accounts for all the evidence, or teaching diagnostic reasoning to novices.
How it can help
This model teaches the tension between simplicity and completeness in any explanatory endeavor. In debugging software, parsimony says look for one root cause rather than assuming three independent bugs. But experienced engineers know that sometimes there really are multiple simultaneous failures. The practitioner wisdom is that parsimony is a starting strategy, not a fixed rule — begin by looking for a single explanation, but don't force-fit complex situations into artificially simple narratives. Knowing when to apply parsimony and when to abandon it is the mark of mature diagnostic thinking in any field.
