Hickam's Dictum (Multiple Simultaneous Diagnoses)

Hickam's Dictum — 'A patient can have as many diagnoses as they damn well please' — is the clinical counterbalance to diagnostic parsimony. Named after John Hickam, a Duke University professor, the dictum recognizes that real patients, especially elderly or chronically ill ones, frequently have multiple simultaneous, unrelated conditions. An 85-year-old presenting with shortness of breath might have heart failure AND pneumonia AND anemia, each contributing independently. The dictum is not a license for diagnostic laziness (listing every possible condition); it's a warning against premature closure — the dangerous tendency to stop investigating once you find one satisfying explanation.

When to use it

When investigating problems in complex systems with multiple potential failure modes, when initial diagnosis seems to explain only part of the picture, when working with complex situations that have accumulated multiple issues over time, or when the elegant single explanation doesn't quite fit all the evidence.

How it can help

This model corrects the overuse of Occam's Razor in complex situations. In organizational troubleshooting, insisting on a single root cause when a system has multiple independent failures leads to incomplete fixes. A company losing customers might simultaneously have a product quality problem, a customer service problem, AND a pricing problem — each requiring its own solution. Hickam's Dictum teaches you to stay open to multiple simultaneous causes, especially in complex systems with many interacting components. It's particularly valuable for avoiding the narrative fallacy — the human tendency to weave a single satisfying story when reality is messier.
