Medical Error Swiss Cheese Model (Reason's Model Applied)

James Reason's Swiss Cheese Model, as applied specifically in healthcare, views medical errors not as failures of individual competence but as system failures where multiple defensive layers (like slices of Swiss cheese) each have holes, and an error reaches the patient only when the holes momentarily align. The layers include organizational culture, institutional policies, supervision, training, protocols, checklists, and individual vigilance.
