Evidence Hierarchy (EBM Pyramid)

Evidence-based medicine organizes evidence into a hierarchy from weakest to strongest: expert opinion and case reports at the bottom, case-control studies above them, cohort studies higher, randomized controlled trials (RCTs) near the top, and systematic reviews and meta-analyses of RCTs at the apex. Each level up provides more protection against bias: case reports can't control for confounders, observational studies are vulnerable to selection bias, RCTs control for both but may lack generalizability, and systematic reviews synthesize multiple RCTs to increase power and reduce the influence of any single flawed study. The practitioner's nuance: the hierarchy is a guideline, not a straitjacket. A well-designed cohort study may provide better evidence than a poorly designed RCT. Some questions (rare diseases, surgical techniques, long-term outcomes) can never be studied with RCTs.

When to use it

When evaluating claims backed by 'evidence,' designing research or data-gathering approaches, deciding how much confidence to place in a finding, arguing for or against a proposed intervention based on the quality of supporting evidence, or distinguishing strong evidence from compelling anecdotes.

How it can help

This hierarchy provides a universal framework for evaluating the quality of evidence in any domain. When someone cites 'research shows...', you can ask: what kind of research? A single case study, a large randomized trial, or a meta-analysis? In business, most 'evidence' for management practices is at the expert opinion or case study level — useful but far weaker than people treat it. In policy debates, understanding the evidence hierarchy helps you distinguish between 'we have a randomized trial showing this works' and 'a few successful companies did this.' The hierarchy doesn't tell you what to believe, but it calibrates how confident you should be.
