Automation Complacency (Ironies of Automation)

Lisanne Bainbridge's foundational insight: automation creates three paradoxes that can make systems less safe rather than more. (1) The irony of skill degradation: automation removes the need for manual skill, but that skill is exactly what's needed when automation fails—so the situations where humans must take over are precisely the situations they're least prepared for. (2) The irony of monitoring: automation makes the task boring enough that humans can't maintain vigilance, but monitoring automation requires sustained attention—the very thing boredom prevents. (3) The irony of complexity: automation designed to help the operator often makes the system more complex, creating new failure modes that require even more skill to manage. Tesla Autopilot crashes exemplify all three: drivers lose the skill of attentive driving, can't sustain monitoring attention, and face a system with complex edge-case behaviors. The 737 MAX crashes demonstrate the complexity irony: MCAS, designed to make the plane easier to fly, created a failure mode that required advanced troubleshooting skill the pilots didn't have.

Provides specific design principles that address each irony. For skill degradation: (1) build regular manual practice into automated operations, (2) design for smooth human takeover, not just smooth automation. For monitoring: (1) give humans meaningful tasks alongside monitoring, (2) implement 'attention checks' that detect disengagement. For complexity: (1) make automation transparent—operators must understand what the automation is doing and why, (2) design failure modes that are simple and recognizable, not complex and surprising. The overarching principle: design automation to support human capability, not replace it. The moment you assume humans aren't needed is the moment you've created the most dangerous system.
