The mechanism: (1) the formal system has limitations or bugs, (2) front-line workers create workarounds to get their jobs done (spreadsheets, manual checks, verbal agreements, post-it notes), (3) the workarounds work, so there's no crisis to trigger formal fixes, (4) the workarounds accumulate and become load-bearing infrastructure, (5) the formal system becomes a facade—the real operations run on the shadow system, (6) when the people who created and maintain the workarounds leave or when the workarounds break simultaneously, the operation collapses. The pattern is ubiquitous: hospital nursing workarounds for electronic health record limitations, factory floor adjustments to flawed production processes, and the legendary Excel spreadsheets that actually run most of the world's financial infrastructure.

When official processes don't match actual practice. When operations seem fragile despite formal systems. When key employees seem irreplaceable for reasons no one can articulate. When a formal system change causes unexpected downstream failures.
