Fat Finger Error (Small Mistakes, Huge Consequences)

A small, often physical mistake—a mistyped number, a misplaced decimal, a clicked wrong button—that cascades into massive consequences because the system lacks safeguards proportionate to the consequences. The 2010 Flash Crash was triggered partly by a fat-finger trade; billion-dollar accounting errors have started as misplaced decimals. Fat finger errors reveal a design flaw: when the cost of a mistake is catastrophic but the mechanism of making it is trivially easy, the system is fragile in a specific, addressable way.

Then engineer proportionate safeguards: confirmation steps for large transactions, review requirements for consequential communications, undo functionality for reversible actions, and rate limits for irreversible ones. The design principle: the difficulty of making an action should be proportional to its consequences. Sending a company-wide email should be harder than sending a DM.
