Capture at the Transition

You reopen the work and spend the first ten minutes remembering where you were. Leave a tiny restart note exactly when you stop, switch places or pass the work to someone else.

Done. Next. Blocker. Risk. Date.

The boundary between sessions is where working context easily disappears. Before leaving it, capture only what the next actor needs to resume: what changed, the next action, what is blocked, what deserves caution and when the note was made. The record carries continuity so memory does not have to reconstruct it.
