Exception Basket

The routine works until an unusual case arrives, then somebody guesses or the case disappears. Define the ordinary limits and give everything outside them a visible review queue with a responsible human and response expectation.

An exception basket separates stable routine work from cases that need a person’s judgment. State the conditions for ordinary processing, flag anything outside them and name who reviews the queue. Track missed exceptions as well as unnecessary flags. A basket only helps if someone can notice, decide and record what happened.
