Put exactly one person's name on each outcome that matters, and label everyone else as either helping or reviewing, so nobody mistakes being included for being in charge.

Your mother comes home from hip surgery and the three of you start a group chat. Someone posts a photo of the discharge sheet. Someone says they will look into physical therapy. Two weeks later you learn nobody booked it, the referral window has closed, and she is walking worse than the day she came home. No one dropped the ball on purpose. All of you read the same message, and each of you heard it as addressed to somebody else.

The cue is a plan where the subject of every sentence is "we," and lots of people are contributing but nobody is closing the loop. The fix: write the outcome as a finished result rather than an activity, "PT scheduled and first session attended by the 14th," not "look into PT." Put one name and one date on it. Sort everyone else explicitly into contributors, who do pieces of the work, and reviewers, who give input. Then give that owner what the job needs, the insurance login, permission to call the clinic. What changes is that the chat stops producing intentions and starts producing appointments. This goes wrong when you confuse owning a task with owning a person's choices; your mother still decides her own care.
