Shared state

Put the facts two people both need in one live place they can each see and update, instead of letting them live in someone's memory, inbox, or private notes.

Your mother comes home from the hospital and you and your brother split the care. He drives her to Tuesday appointments, you handle medications and insurance. Three weeks in, the cardiologist asks what dose she is on and neither of you is sure, because he wrote it on a receipt in his truck and you changed it after a phone call you never mentioned. A refill lapses. She sits at home for four days without a pill she is supposed to take daily, and each of you thought the other had it.

Shared state means giving both carriers one current, permissioned source of truth. The cue is a question whose answer exists only in one person's head, phone, or inbox. Open one document both of you can edit. Put in the live facts: current medications and doses, upcoming appointments, what each doctor last said, insurance status, open questions. Write who may change what, so dose changes go through whoever talks to the prescriber. Update it the day something changes, not weekly. What changes is that nobody has to reconstruct the truth from memory, and gaps become visible instead of silent. This is wrong when the facts barely move or the update cost exceeds the coordination it saves, and it is a real risk when the information is private enough that broad access harms the person it describes.
