S.MISCAST.M — Memory/state failure
Your system breaks because important information lives in one person's head, or is scattered, stale, or lost.
Your mother sees three doctors, and each one asks what medications she takes. The real list lives partly in her purse, partly in your sister's texts, partly in a portal none of you can log into. Last month a new prescription clashed with an old one because nobody had the full picture. The information exists. It just lives nowhere in particular, and the version anyone can find is stale.
This is a diagnostic label, not a fix: it names situations where the breakdown is memory and state, meaning important information is trapped in one head, scattered across places, duplicated, out of date, or lost. The cue is a recurring question no one can answer quickly, or two sources that disagree. To run it, ask three things. What must stay current for this to work? Who can retrieve it at the moment they need it? Which copies are stale right now? Answering those points you toward the remembering moves: capture, retrieve, synchronize, retire. When it works, you stop blaming people for forgetting and start fixing where the information lives. Do not reach for this diagnosis when the facts are all present and current but people disagree on judgment; that is a different failure shape.
